Quality Care Services
We provide the following services for residents in Northeast Georgia (Clarke, Oconee, Barrow, Jackson, Madison, Oglethorpe, Elbert, Hart, Walton, Morgan, Greene, Hall, Banks and Franklin counties).
- Telephone and office consultations
- Comprehensive in-home assessment—our care managers assess the older adult’s physical abilities, emotional and mental status, legal and financial status, as well as safety assessment of the home. We develop a written report of our observations, assessment of needs, and recommendations for current and future care and services. The individualized plan of care includes recommended services and vendors, the costs, and how to access the services. We review the assessment and plan of care with the older adult and his family members and remain available to assist with implementing the recommendations.
- Care Coordination—our care manager assists with service arrangement and coordination amongst the service providers. The care manager will call and visit the older adult monitoring for any change in his needs while adjusting the care plan as needed. The care manager keeps the family, physicians, and other service providers up to date on the status of the older adult. If relocation to another level of care is needed, the care manager can help with transitioning and moving the older adult.
